I see (or rather don't see) a few things I might put on an OD&D sheet that aren't here (Spells, frex, but that could be an example of "Other Abilities" and make the sheet more universal), but I can't begin to guess what kind of omission would be intentional vs. unintentional ... (my only solid guess was "hit points," and someone took care of that for me) :)

what I've heard is that the ability scores are ordered the way they are because that's the order of the prime req. for each class in the order they were created:
STR- fighting man INT- Magic User WIS- Cleric DEX- Thief CON- no class CHA- no class
